Why we start with the medical card

27%

of driver out-of-service violations at Roadcheck 2026 were for one thing: no medical card.

Alerts don't renew med cards. We follow through.

We warn you weeks ahead. We send a list of National Registry examiners near the driver. Then we confirm the new card from the MVR you pull or upload.

Drivers without a CDL: confirmed from the examiner's certificate.

What we run

  • A random testing pool.
  • A C/TPA in the Clearinghouse.
  • Someone to watch every medical card.
  • Someone to keep every driver file.

These aren't four jobs. It's one company.

4 draws a year

Random pool

Unannounced draws every quarter at FMCSA's 2026 rates: 50% drug, 10% alcohol.

$69 / $59

DOT tests

Drug and breath alcohol tests through Pact's lab network at Quest collection sites. Or keep your own lab and MRO.

#2 & #3

Clearinghouse queries

In FMCSA investigations in fiscal year 2025, missed pre-employment and annual queries were the #2 and #3 most-cited violations. We run yours on schedule.

Getting covered

Send a roster today. Be covered this week.

  1. Day 1

    You send the roster.

A spreadsheet or a photo of a list.

  2. Day 1

    Drivers go in the pool.

Your certificate of enrollment is issued.

  3. Week 1

    We load the rest.

Queries, med card dates and driving-record reviews.

Start your free month

Day 1 starts once the agreement is signed and payment is set up. Queries start after you name Pact your C/TPA in the Clearinghouse and buy a query plan. FMCSA requires both.

New carrier? Would you pass your safety audit today?

Sixteen things fail a new carrier's safety audit. Six are about testing and medical cards, and Pact tracks all six.

Tick the ones that are true. Nothing is sent anywhere.

See all 16, and what happens if you fail
  • 382.115 No drug and alcohol testing program
  • 382.305 No random testing program
  • 382.201 Using a driver at 0.04 alcohol or above
  • 382.211 Using a driver who refused a test
  • 382.215 Using a driver who tested positive
  • 391.11(b)(4) Using a physically unqualified driver — the medical card
  • 383.3 / 383.23 Driver without a valid CDL
  • 383.37(b) Operating with a disqualified or invalid license
  • 383.51(a) Allowing a disqualified driver to operate
  • 387.7(a) Operating without required insurance
  • 387.31(a) Passenger carrier without required insurance
  • 391.15(a) Knowingly using a disqualified driver
  • 395.8(a) No records of duty status
  • 396.9(c)(2) Running a vehicle that's out of service
  • 396.11(a)(3) Not fixing out-of-service defects
  • 396.17(a) Running an uninspected vehicle

You get 60 days to correct it — 45 if you haul passengers or hazmat. Miss that and FMCSA revokes your registration and puts your operation out of service. 14 of the 16 fail on a single violation; logs (395.8(a)) and periodic inspections (396.17(a)) fail at 51% of the records checked.

What we hand you: an audit pack that prints those six lines with your own drivers' names against each one, and says plainly where you still have a gap.

Compare with the market, and what else can show up on a bill

First year, all in: what a fleet pays

Type of service5 drivers15 drivers40 drivers
Random pool only (you keep the files, run the queries, chase the med cards)$329–850$689–950$1,589–2,400
Pact (pool, Clearinghouse queries, med cards, MVR reviews, audit binder)$1,577$4,731$12,616
Done for you (a service runs the files; setup fees common)$2,562–6,187$7,426–9,287$12,387–19,311

Plainly: a pool-only consortium costs much less than Pact. It runs the random pool and leaves the rest of the file to you. Against services that do the work for you, Pact costs less at 5 and 15 drivers and about the same at 40. Pact's numbers are year one at $25 a driver a month with the first month free, plus the expected random tests at 50% drug and 10% alcohol at $69 and $59. Others are each seller's published price plus tests where the price leaves them out. Left out for everyone: pre-employment tests, post-accident tests, physicals, MVR state fees and FMCSA's $1.25 query fee. Prices read from sellers' own pages September 29 and October 15, 2026.

From each seller's own published price page, read September 29 and October 15, 2026. We keep the list on file and recheck it monthly; ask and we'll send it.

What else can show up on a bill

  • After-hours on-site collection, arranged in advance: billed at cost, usually $450 and up plus collection fees. Until it's arranged, drivers go to the nearest open collection site.
  • A directly observed collection, when one is required: $50 extra, at a site that offers it. Pact finds one near the driver.
  • Clearinghouse query fees you pay FMCSA directly. FMCSA doesn't let a C/TPA buy them for you.
  • Driving records you pull are billed by the company you pull them from.
  • Month to month: 30 days' written notice and you're out, with your records, exported free.

More answers in the guides

How fast can I be covered?

Once your payment method is set up, send the roster and your drivers go into the pool the same day, with your certificate of enrollment issued so you have something to hand a broker or an inspector. The rest — queries, medical card dates, driving-record reviews — we load in the first week.

Can I switch providers mid-year?

Yes. Your last Clearinghouse query, medical card and driving record dates come across with the roster, your previous provider's test records go into each driver's file, and we pick up the calendar from there. Your history comes with you.

We already test with Quest (or another lab). Do we have to change?

No. Keep your lab account and your Medical Review Officer — DOT rules don't require you to use your C/TPA's lab. You sign a one-page letter asking your MRO to copy us on each result, as 49 CFR 40.349(c) allows, and we run everything else: the random pool, Clearinghouse queries and reports, medical cards, driver files, the MIS report and your records. Your lab tests your drivers; we keep the records an auditor asks for. Or test through Pact's lab network (Quest collection sites, DRS as Medical Review Officer): testing starts once your lab account is set up, usually 3 to 4 business days after you sign.

How will I know when one of my drivers is picked for a random?

Pact draws once a quarter, on an unannounced day, and emails your designated employer representative the same day: how many were picked, not who — the names stay in the portal, so a forwarded email can't tip a driver off. Each selected driver stays on your Today page until you send them, and anything unsent brings another email at 30, 14, 7 and 3 days before the quarter ends. When you send someone, the portal records when, how and where. Owner-operators get no warning at all: Pact sends you to the test, as the rules require (49 CFR 382.305(k)(1)).

What happens if I'm audited?

You open the portal and print the binder. Every driver has one dated file: Clearinghouse queries with results, test records with retention dates, the medical certification history, the consent on file, the policy signature, and every notice we sent. It prints with a letterhead and an attestation, and it lists anything we don't hold so nothing reads as verified that isn't.

Do my drivers have to download an app?

No. They get a one-time link on their phone. It opens their page — photo a new medical card (for a CDL driver, we confirm it on the state driving record), sign the policy, fix their phone number — and then it expires. No account, no password, nothing to install.

Who sees my drivers' test results?

Your designated employer representative, who gets each result from the MRO, and Pact, which keeps a copy for your records. No other employer or third party sees them without the driver's written consent, except where the rules require or allow it — a DOT audit, for example (49 CFR 40.321, 40.331), and a driver can request their own records in writing (40.329). That's Part 40, Subpart P, and it's not negotiable. Drivers have their own page for their medical card and policy signature; it cannot show test results or random selections, not even their own.

What if a driver tests positive?

A licensed Medical Review Officer reviews the result and tries to speak with the driver before verifying it — the rules require that (49 CFR 40.131), it's not a courtesy. If the result stands, the driver is immediately prohibited from safety-sensitive work, the MRO reports it to the FMCSA Clearinghouse, and the driver must be given a list of qualified Substance Abuse Professionals before the return-to-duty process can start. The portal sends the driver a list of qualified SAPs near them; if Pact has none on file for that state yet, we tell you and help you find one. We'll tell you exactly what you can and can't do next, in writing.
