Every caregiver file, checked for you.
Pact Compliance Review checks every upload, usually within minutes, 24/7. Caregivers upload from their phone; Pact tracks background checks, training hours, TB and CPR, and runs a monthly exclusion check.
The sample is a nursing home. Home care, home health and hospice files work the same way.

What your state asks for
Every state writes its own caregiver rules. A few examples, from each state's licensing rules:
- Georgia: registry checks (nurse aide, sex offender and OIG) before a fingerprint state and FBI check; 40 hours of training for personal care assistants; 8 hours a year after the first year; TB screening at hire and every year (Ga. Comp. R. & Regs. 111-8-65).
- Washington: 75 hours of entry-level training within 120 days of hire, with orientation and safety training before any care, and 12 hours of continuing education a year (RCW 74.39A.074).
- Florida: Level 2 background screening for home health, nurse registry and companion staff, and 40 hours of training or the state competency test for home health aides (F.S. 400.512; Fla. Admin. Code R. 59A-8).
- Pennsylvania: a criminal history report at application or within the prior year, and a competency exam or approved training before a direct care worker is assigned (28 Pa. Code 611).
Pact adds two state rechecks to your list for you: a Texas agency gets the yearly nurse aide and employee misconduct registry search for its aides (26 TAC 558.247), and a Florida agency the level 2 background rescreen every 5 years (F.S. 408.809). Other states' rules go on your list as items you add: limit each one to certain roles and set how often it renews.
How it works
Your state, your list
Start from a ready-made healthcare list and set it to your license: limit items to certain roles and add your own.
Caregivers upload from their phone
Each caregiver gets a link to their own page: what's still needed, when each item runs out, and a camera button. No app, no password.
Every upload checked for you
Pact Compliance Review checks each one against written criteria, usually within minutes, 24/7. If an upload can't be used (blurry, the wrong document, expired by its printed date, or someone else's name), the caregiver is asked for a new copy by email (and by text once texting is on for your account), with their own link. You still see everything, and you can change any decision.
Reminders before anything expires
Caregivers are reminded 30, 14 and 7 days before anything expires, and send the renewal from their phone. If someone ignores the 30, 14 and 7-day reminders, the manager or director of nursing you name gets one email with the list. Reminders never ask for something already approved and good for more than 30 days.
"Please retake," right away
Instant feedback on the phone: a blurry photo, the wrong document, an expired card or someone else's name gets "Please retake" and the reason while the caregiver still has it in hand. Pact Compliance Review checks uploads, usually within minutes, 24/7.
Licenses and registries, checked for you
We check every license and aide registry for you. Pact's staff look each one up by hand on the free public lookup sites and record when, where and the result, with a screenshot or PDF of the lookup when the site allows one.
TB rules and quick fixes
TB items follow CDC's 2019 rule (a test at hire, yearly TB education, a retest after an exposure) unless your state asks for more, as Kentucky does with a yearly risk assessment. Fix a date yourself, with a note in the change log, and export staff files by department or role as CSV or Excel.
One email every Monday
Every Monday you get one email: who's expired, who's coming up, and what we handled for you.
Your staff list, loaded for you
Send us your staff list in any format. We load it for you. Excel, CSV, PDF or a photo of a printed list all work, and you can upload an Excel file yourself.
Monthly roster update
Each month, upload your payroll or HR export (Paycom, ADP, Paylocity or any other, as CSV or Excel). Pact shows who's new, who's no longer on payroll and what changed, and nothing changes until you confirm. There's no direct connection to your payroll system: you upload the file.
Monthly exclusion checks
Every active caregiver against OIG's exclusion list and the Medicaid lists of 25 states automatically (the rest by a monthly hand-check task in your portal, with the proof saved), monthly and whenever you add someone, with a dated record of each check. In 2026 so far, OIG has excluded 705 nurses and nurse aides and 273 personal care workers (OIG's list, updated October 9, 2026). Check up to 50 staff now.
See the portal in 2 minutes: a click-through of a sample nursing home (home care files work the same way), no login.

Common questions
Does it fit an agency that isn't Medicare-certified?
Yes. You start from a ready-made healthcare list and set it to your own state's license and your own policies: limit items to certain roles, change how often anything renews, and add your own. Medicare home health agencies and hospices get the federal aide rules on top, set by your care setting: the RN competency evaluation before the first visit, 12 in-service hours a year and each aide's on-site supervisory visit (42 CFR 484.80 for home health aides, 418.76 for hospice aides): see staff files for Medicare home health and hospice.
Do caregivers need an app?
No. Each caregiver gets a link to their own page on their phone, with no app and no password. They see what's still needed, take a photo or upload a PDF, and see when each item runs out.
Which exclusion lists do you check?
OIG's federal list every month, plus the Medicaid exclusion lists of 25 states automatically, for the states you operate in. For 20 more states and DC, Pact can't read the state's list automatically (Texas, Florida, California, Illinois, Minnesota and others), so the rest are done by a monthly hand-check task in your portal, with the proof saved: you search each one that applies to you on the state's own site. Five states (Oklahoma, South Dakota, Utah, Virginia and Wisconsin) keep no list of their own and use OIG's list, which Pact already checks. A possible match comes to you to confirm.
Who reviews the documents?
Pact Compliance Review checks every upload, usually within minutes, 24/7. Pact's own staff check each upload against written criteria, and ask the caregiver for a new copy when one can't be used. You still see everything, and you can change any decision. An automated system reads each upload; a Pact reviewer confirms it. It never approves anything on its own, and background checks, drug screens and test results are never sent to it.
What does it cost?
$10 per active staff member a month, with a $250 monthly minimum. The first month is free (no monthly fee for the rest of the month you sign or the next full month), there's no setup fee, and it's month to month. Caregivers who have left aren't billed. Background checks and drug tests you order are billed when ordered.
Do you run the background checks?
Pact tracks every check on file with its date. Your state's own required checks (fingerprints, state registries) are done through the state's process; Pact keeps the dated result in each caregiver's file and reminds you when a recheck is due. You can also order a name-based check online (Standard $35, Premium $70). Orders are taken now and start when Pact's Checkr account opens; Checkr then emails the person.
What do we hand the surveyor?
Pick the people and download one packet: a summary page plus every approved file, in one zip file. Every download is logged.
Can you update our caregiver list from payroll?
Yes. Once a month, upload the export from Paycom, ADP, Paylocity or any payroll or HR system, as CSV or Excel. Pact shows who's new, who's no longer on payroll and what changed. Nothing changes until you confirm, and nothing is deleted. Caregivers marked as left stop being billed.
Is there free training for our aides?
Yes. Pact's free in-service courses cover topics like infection control, abuse prevention, dementia care and patient rights, each with a quiz and a printable certificate of completion. They aren't continuing education credit and aren't approved by any state; your agency decides whether they count toward your hours.
