How Much Does a DOT Drug Test Cost in 2026?
Short answer: a DOT urine drug test costs about $60 to $99 from the online sellers we checked in October 2026, and a DOT breath alcohol test costs about $59 to $99. The drug test price should cover the collection, the certified lab and the Medical Review Officer (MRO) review. Extra fees can apply for observed collections, on-site collections and Clearinghouse queries. This guide explains what you pay for, what the sellers charge, and what the rules say about who pays.

What a DOT drug test price should include
A DOT urine drug test is more than a lab fee. It has three parts, and a fair price covers all three.
If a seller quotes a low price, ask whether the MRO review is included. Every DOT drug lab result goes to an MRO for review.
- The collection: a trained collector at a collection site takes the specimen and fills out the custody and control form (40.3).
- The lab: a U.S. lab must be certified by HHS under the National Laboratory Certification Program to do DOT drug testing (40.81).
- The MRO review: a licensed physician reviews the lab result and looks at medical explanations before the result is reported (40.3).
What a breath alcohol test involves
A DOT alcohol test is a separate test with a separate price. A Breath Alcohol Technician (BAT) runs it on an evidential breath testing device approved by NHTSA (40.3).
You do not need an alcohol test every time you need a drug test. Pre-employment alcohol testing is optional for FMCSA employers. If you choose to do it, you must test every new driver the same way (382.301(d)).
Alcohol tests do come up in random testing, after certain accidents, and for reasonable suspicion (382.303, 382.305, 382.307).
Published prices we checked (October 2026)
We loaded each seller's own website in October 2026 and wrote down the listed price. We describe the sellers instead of naming them. Prices change, so check a seller's current price before you order.
- An online consortium with flat test prices: $60 for a DOT drug test and $60 for an alcohol test. Its price table says MRO review is included with drug tests.
- A second online DOT consortium: $69 for a DOT drug test and $59 for a breath alcohol test, listed as member pricing.
- A national drug testing company: $75 for a DOT urine drug test.
- A national online test seller with its own random pool: $99 for a DOT drug test and $99 for a DOT alcohol test. Members of its random pool pay $79 per DOT drug test.
What that means for your budget
Across these four sellers, a single DOT drug test runs about $60 to $99. A breath alcohol test runs about $59 to $99 at the three that list one.
Several sellers charge less to members of their consortium or random pool. If you also need a random pool, compare the membership fee plus the member test price, not the test price alone.
To see how many tests you will need, start with the 2026 FMCSA random rates. They did not change for 2026. You need random drug tests equal to at least 50% of your average number of driver positions, and random alcohol tests equal to at least 10% (382.305). A fleet that averages 10 drivers needs at least 5 random drug tests and 1 random alcohol test this year, plus a pre-employment drug test for each new driver.
Extra fees to watch for
The test price is not always the whole bill. These are the add-ons that come up most often.
- Observed collections: return-to-duty and follow-up drug tests must be collected under direct observation (40.67(b)). Some other events also require one, such as signs of tampering or a specimen temperature out of range (40.67(c)). The first online consortium above lists a $40 observed test add-on fee.
- On-site or emergency collections: having a collector come to you is priced separately. The national online test seller above lists a short-notice post-accident site visit at $899.
- Clearinghouse queries: FMCSA charges a flat $1.25 per query. You buy a query plan in the Clearinghouse yourself. A C/TPA may not purchase a plan for you. Some C/TPAs list their own query fee too; one online consortium lists a Clearinghouse query at $5. Ask what you will pay on top of FMCSA's fee.
- Membership: at most sellers, the consortium or random pool fee is billed apart from tests. One online consortium says testing is separate from its enrollment fee.
Who pays for a DOT drug test
The DOT rules do not say. ODAPC, the DOT office that runs the testing rules, says the regulations are silent on who pays. Payment can be based on an understanding between the employer and the employee, including job applicants.
There is one firm limit. If a driver asks for a test of the split specimen after a positive result, the employer must make sure that test goes ahead in a timely way, even if the employer has to pay for it. FMCSA says the employer may seek reimbursement through company policy or a collective bargaining agreement.
Owner-operators are both the employer and the driver under the rules (382.103(b)), so in practice the bill is theirs.
Pact's DOT test prices
Pact charges $69 for a DOT drug test. That includes the collection, the certified lab and the MRO review. A DOT breath alcohol test is $59. When a directly observed collection is required, it is $50 extra.
Tests run through Pact's lab network: Quest collection sites, with DRS as the MRO. If you already have your own lab account, you can keep it, and Pact does not charge you for those tests.
Clearinghouse queries are still $1.25 each, paid to FMCSA from the query plan you buy.
Common questions
How much does a DOT drug test cost in 2026?
About $60 to $99 at the four online sellers we checked in October 2026. Pact's price is $69, with the collection, certified lab and MRO review included.
How much does a DOT breath alcohol test cost?
About $59 to $99 at the three sellers we checked that list one. Pact's price is $59.
Who pays for a DOT drug test, the employer or the driver?
The DOT rules are silent on it. ODAPC says payment can be based on an understanding between the employer and the employee, including applicants. An employer may not hold up a split specimen test because the employee has not paid in advance.
How much is a Clearinghouse query?
FMCSA charges a flat $1.25 per query. You pay it through a query plan that you buy in the Clearinghouse. A C/TPA may not buy the plan for you.
Do I need an alcohol test before hiring a driver?
No. Pre-employment alcohol testing is optional for FMCSA employers. If you choose to do it, you must do it for every new driver (382.301(d)).
Why does an observed collection cost more?
It takes extra steps at the collection site, so many sellers charge an add-on fee. It is required for return-to-duty and follow-up tests and in some other cases set out in 40.67.
